How to make a referral

Any Dorset agency may refer to the DCFCT service. These agencies may include CAMHS, GPs, Social Services, schools, courts and Youth Offending Teams. Families may not apply directly but can do so through any of these agencies. There is a standard referral form that needs to be completed. Please contact us to ask for a copy if you require one or download a form from this website. Note that referrals must meet our criteria, and provision of counselling is subject to availability of funds at the time of application.
